Perhaps a little more information.
I am the type to take life as it is presented. I enjoy things as they are, when they are. I carry with me a strong desire to improve myself. I carry a journal with me that also acts as a date book. I don't miss appointments without an immediate and pressing need stopping me. I fulfill my obligations with the exception of missing house chores for a few days. For these things I am glad.
I also carry with me a desire to refine my knowledge, my skills and my body. Those things require a long term commitment. A commitment to study, to practice, and to healthy lifestyle choices. Despite these desires, and often a strong showing for the first few months, I have been unable to attain my degree, to settle on a single expertise or to regularly work out or avoid smoking, drinking and rich foods.

Why is that? What make you lose interest or commitment?

Sometimes what is new can be exciting at first but then the originality of it goes away and you are left with the reality of what you're doing. If you don't like that then you are destined to have problems

Maybe it isn't what you thought it would be and if it is maybe you thought wrong? If you are determined to continue but find a lack of willpower try changing it up a bit. Maybe approach what you are doing from a different angle, take on different responsibilities, learn in a different way etc with luck you will find one of those more suiting :)

As far as 'urges' go it is simple to say that it is 'mind over matter' (of sorts). You could try finding a healthy replacement such as meditation or drinking water etc, maybe even standing up walking around the house or going for a walk. Just to distract yourself. Overtime you should forget the need by knowing you will never satisfy it

Do it in steps. Try for 1 day then 3 then 5 then a week etc, build up...

O those goals. You're human. Everytime you fall of the horse get back on. Don't beat yourself up just start back up again. Keep trying. Relax and enjoy the trip. Don't worry about the destination. Be happy.
