As someone who works in healthcare, let me throw this in:

I understand this as the difference between regarding the healthcare professional as an authority or an employee, or somewhere in between.

Might I suggest that is also important to talk about the role of the health care recipient as either passive or active. We regard healthcare as something that is done to us rather than something we do.

We approach our doctors with problems we already have, which definitely sets the stage for the conversation. Just to be clear, I am as guilty of this as anyone. But I suggest that if we were active participants in our health, partners if you will, if would change the whole discussion.
